A group quote is easy, just provide us with following census information including your plan matrix:

• DOB or age:

• Job Title or Position:

• Class of Employee: (if you are looking for a different classes of insurance)

• Salary: (for long and short term disability)

• Marital status and number of dependents: (for dental)

• Anything special about the group

• Locations of group

For your plan matrix we need to know:

• How many classes you require and their makeup (management, supervisors, laborers)

• Schedule of benefits (how much single limit or each class)

• Elimination and benefit periods for DI (60, 90, 180 / 5 year, A65)

• Retirement age reduction (age 65)

• Length of premium guarantee required (one year or two year)

• Anything special required
